Agreement and Waiver of Liability

(Child Participant)

The following Waiver applies to all activities and events associated with WyoBraska Extreme Cheer, Inc., (hereinafter "The Program"). Please complete the following, being sure to carefully read and sign after each paragraph.

As the legal parent/guardian of the above named child, I hereby acknowledge that my child is voluntarily participating in sports activities organized by The Program. I understand that these activities involve inherent risks, including but not limited to bodily injury, property damage, and death, which may occur as a result of my child's participation. Therefore, I agree to the following terms:

1. My child is participating in activities and events associated with The Program, during which my child will receive information and instruction about safe participation. I understand and acknowledge that my child's participation in these activities involve inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all risks associated with my child's participation in these activities.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my child's participation in The Program's activities and events. I represent and warrant that my child is physically fit and has no medical condition that would prevent their full participation in these activities and events. I agree to inform the coach/instructor of any physical limitations, physical discomfort, and/or injuries before or during classes, and I take full responsibility for nondisclosure.

3. I hereby release and forever discharge The Program, its directors, officers, agents, employees, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that my child may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to my child, whether caused by the negligence of the Released Parties or otherwise.

4. I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my child's participation in the sports activities and events associated with The Program.

5. I authorize The Program, its agents, employees, and volunteers, to obtain or provide medical treatment for my child in the event of any injury, illness, or other medical condition that may occur during my child's participation in the sports activities.

6. I have read this Waiver fully, understand its terms, and understand that I am giving up substantial rights, including my right to sue the Released Parties for any injury, death, or damages that may result from my child's participation in the sports activities organized by The Program.

By signing below, I hereby acknowledge that I have read and fully understand this waiver, and I voluntarily agree to its terms.
